Information Sessions for Individuals and Families Receiving I/DD Services - Western Region

5/1/2012 - Individuals and Their Families Receiving Medicaid or State-funded Services for Intellectual and Developmental Disabilities

Smoky Mountain Center (SMC) and the SMC Consumer and Family Advisory Committee (CFAC) invite you to attend a free information session about upcoming service system changes that may impact you.

Topics include:
  • Moving from Case Management to Care Coordination

  • Changes July 1 from CAP/I-DD to Innovations Waiver

  • Who will help you with the transition?
