Attached you will find a Self Assessment Tool to complete prior to August 15, 2012. In completing the Self Assessment you will have a better understanding of the requirements of monitoring as we move into the Medicaid Waiver. SMC Provider Network Department will be completing visits within the next 12-18 months to review for compliance to the Self Assessment Tool.
