ATTENTION SMC I/DD Targeted Case Management Providers: CAP-MR/DD Plan Transitions to NC Innovations - In the next week, I/DD Care Coordinators from SMC will contact your agency's Targeted Case Managers who currently work with CAP-MR/DD consumers. The purpose of this contact is to begin the transition of current CAP-MR/DD plans to NC Innovations services. I/DD Care Coordinators will work with your staff to obtain copies of current CAP-MR/DD plans for transition meetings with participants/families/legally responsible persons. We welcome input from current TCMs as we prepare for those transitions. At this time, SMC is in no way implying that we are going to begin care coordination activities; just that we need to begin the process of transitioning the plan services.
Caseloads have been assigned and we are aware that there will be planning meetings for June birthdays (for services effective July 1, 2012). We would like the appropriate I/DD Care Coordinator to participate in those planning meetings.
